EXPEDITED BIRTH CERTIFICATE FOR INDIVIDUALS BORN OVERSEAS WITH AMERICAN PARENT(S) AT THE TIME OF BIRTH

If you were born overseas by a U.S. parent(s) and would like to request a certified copy of Report of Birth OR Certification of Birth (replace or Amend a Consular Report of Birth Abroad – CRBA), please completely fill out and sign the following forms:

You can also use these forms for obtaining a certified copy of Report of Death, Panama Canal Zone Birth or Death Certificate, Certificate of Witness of Marriage, Certification of No Record.

  • If you want the Consular Report of Birth Abroad Apostilled for International Use, please send the following:
    A. A notarized Letter of Authorization
    B. A notarized copy of the ID of the person who completes and signs this form.

NOTE: On January 3, 2011, the Department of State began issuing a new Consular Report of Birth Abroad (FS-240). You may request multiple copies of this document at any time. As of December 31, 2010 the Certificate of Report of Birth (DS-1350) is no longer issued. All previously issued FS-240 and DS-1350 documents are still valid for proof of identity, citizenship and other legal purposes.

To Amend a Consular Report of Birth Abroad (FS-240), You Must Submit:

  • A notarized written (or typed) request detailing the amendment or correction needed
  • Certified copies of documents justifying the amendment requested (Examples: foreign birth certificate, marriage certificate, court ordered adoption or name change, birth certificates of the adopting or legitimating parents, affidavits, etc)
  • The original FS-240, or A replacement FS-240, or A notarized affidavit stating the whereabouts of the original FS-240
  • Authorization Letter (please download the form and fill out completely) – this letter must be notarized.
  • Travel the World Visas Service Order Form (please download the form and fill out completely)
  • Notarized copy of the applicant’s identification (driver’s license)
